Knights of Columbus - Dayton District at 267 Bainbridge Street, Dayton Ohio, OH 45402-2208 US - Volunteer Certification Information

1.  Each Knight, Auxiliary Lady, and Squire desiring to be a Volunteer, are required to complete a VA Form 10-7055 (May 2007).  This form will be kept on file at the VA Medical Center Voluntary and Recreation Services (VAVS) Office in building 305.

2.  This form can be completed by each individual volunteer prior to event (Thursday and Fridays) or upon arriving to help with a sponsored Knights of Columbus event at the VA Medical Center.

3.  You may also be expected to complete a fingerprinting request form by the VAVS office of the VA Med Center.  This is accomplished by the Police/Security Office, of the VA Medical Center.  This will facilitate the issue of a VA Medical Center Volunteer ID Card, and a Volunteer Parking permit.

4.  A compiled roster of all Knights of Columbus Volunteers will be maintained by the VA Medical Center Voluntary and Recreation Services (VAVS) Office and the Knights of Columbus VA Director/VAVS Representative.

5.  Volunteer Hours will be accounted for on a Monthly Compilation Report by the VA Medical Centers Voluntary and Recreation Services (VAVS) Office.  A Copy of this report will be made available to the Knights of Columbus VA Director (VAVS Representative). There are recognition programs in place for achievement of Volunteerism and accumulative volunteer hours.

6. All Donations, monies or items of value, must be recorded on Dept of Veteran Affairs- Dayton VA Center DONATION FORM, and signed by a staff member Voluntary/Recreation Services.  This includes Cookouts, Bingos, Tournaments, Clothing items, Gift bags, ect.

7.  The Knights of Columbus and the VA Medical Center appreciates the contributions of your time, your services, and contributions given, to provide a higher quality of total services to our Veterans staying and having to use the Dayton VA Medical Center, and the Services it provides.
