Dear Parents,
Please find attached a registration form for the fall Parish Religious Education Program
here at Sacred Heart. If you have a children) who will be returning to our program or
attending for the first time this fall, please take a few minutes and fill out both sides
of the form and return it by September 1, 2009 to

If your child will not be returning please
let me know. You can either call me at home or on my cell phone. This information is
needed in order for me to order the proper number of books and supplies for this year.
If you are no longer a member of the parish please return the form and mark it
"NO LONGER MEMBER " this will help us update our files.
Children entering the program must be 6 yrs. of age by the 30th of September.
If you have any questions please feel free to call me at home any evening.
