Below are links to the forms you will needs for any youth ministry activity you will be hosting along with a brief explination of what each form is: Form A - Parent/Guardian/Conservator Permission and Liability Waiver This form is to be completed by the parent of a youth participant. It only needs to be completed once a year and is kept on file in the youth office. A copy of it must be brought to every event in which the youth participates. This form needs to be notarized only in the event the youth is going to be participating in an event out of state. Form B - Consent To Participate/Consent For Emergency Treatment This form must be completed for every event the youth participates in and a copy must accompany the youth to and from the event. Form C - Adult Release and Waiver of Liability This form is to be completed by each adult volunteer that works with the youth program. It only needs to be completed once a year.
