Pinellas Hope Respite Services
What we do: Pinellas Hope Respite Services is a pilot program that provides temporary emergency shelter for individuals without homes being discharged from the BayCare system who are too medically frail to return to the streets but do not require further hospitalization or skilled nursing facility care. Primary program objectives include establishing or follow up visits with a primary care provider, enabling continuity of care of discharge condition by providing transportation, case management and advocacy, and financial assistance with Home Health visits and medication costs. Clients Served: 44 (FY 09/10) start up year on three year grant.
Sustainability: Our program is currently funded by a three year collaboration with BayCare System and the Allegany Franciscan Ministries. As part of the larger Pinellas Hope community this small program can provide crucial services to our most vulnerable adults while working toward increased health education for all residents in camp and access to basic medical services that can decrease further hospitalizations and emergency room visits. Program expansion may include a modest fee-for-service for other already interested medical providers.
Who we serve: Catholic Charities Respite Services is currently limited to serving adults without homes that are being discharged from a BayCare facility.
Our service area includes: Pinellas County.
If you want to help: Donations to the larger Pinellas Hope Community are welcome and benefit the clients in the Respite Services Program. Clothes, new underwear, blankets, batteries and personal grooming supplies are needed as well as water-proof wallets, notebooks and folders to help organize residents working toward self-sufficiency and stable housing. Volunteers are needed for a variety of tasks; from grant funding research to direct client assistance with phone calls and form completion, or clerical and computer assistance with confidential client files. A medical background is not necessary but we certainly welcome anyone with nursing, social work, mental health or medical training.
