Medication Administration 
Use this form if you have a medication that our Nurse or Nurse's Aid will need to dispense or hold for your child. Please refer to the Student Handbook for more detail under the 'Medication' section. FYI: You must have parent AND physician signature to give over the counter and prescription medications at school!
