Students should have exceptional achievement in one or more of the following areas: academics, athletics, performing arts, church, or community service. Forms will be reviewed and submitted in the bulletin on a semi-annual basis. Please consider nominating a young parishioner and help us promote the hard work and accomplishments of our youth who are using their God given talents and treasures. Please pray for them that they will continue to use their talents to make Christ alive in the world. (Please note: Completion and submission of this form does not necessarily ensure inclusion in the bulletin)

In order to celebrate the accomplishments of our young parishioners, as well as, enhance unity within the Saint Sebastian community, the Faith Formation Advisory Board is asking that parishioners consider submitting the names of children (grades 6-12) who are members of our Parish for review as part of a student recognition program.
