2. School-age child of poor families
3. Persons With Disabilities (PWD)
(socially disadvantaged persons)
Individuals in crisis situations (ICS):
3.1 sickly persons
3.2 stranded persons/strandies
3.3 bereaved poor families
3.4 disaster victims
WHAT SERVICES ARE AVAILABLE?
1. Community/ Family Enrichment Services: Group Seminars on Value Formation
2. Educational Assistance/ Scholarship
3. Livelihood Assistance
3.1 Capital Loan for family enterprise on a roll-back scheme or micro-financing
3.2 Home-based income-generating production units: Tie-up with financing agencies/ groups
4. Assistance to Individuals in Crisis Situation (AICS) for:
4.1 medicines/ hospitalization
4.2 food and transportation expenses for stranded persons
4.3 food and homelife supplies for disaster victims
4.4 burial expenses
5. Psycho-social/ spiritual guidance and counseling
6. Referral services for other needed services
HOW TO AVAIL OF SERVICES?
1. Apply personally at the office of San Lorenzo Ruiz Charity (SJDMC) Inc., or thru the authorized agency officer/ welfare officer;
2. Fill-up prescribed application form;
3. Application is validated thru interviews and/ or home visits by the designated welfare officer/ social worker, as necessary;
4. Eligible applicants shall be provided the appropriate services as validated and approved by the Director or designated approving officer. Other needs of applicants shall be referred to other agencies/ groups/ individuals for appropriate action;
5. All services received shall be properly acknowledged by recipients thru appropriate documentation.
