Thursday, October 15, 2009 (6:30-7:30pm Returning Players Only)
Thursday, October 22, 2009 (6:30-7:30pm)
Sunday, October 25, 2009 (1pm-2pm)
Thursday, November 5, 2009 (6:30-7:30pm)
Sunday, November 8, 2009 (1pm-2pm)
Thursday, November 12, 2009 (6:30-7:30pm)

Basketball Registration Fee:
Fees: $65.00
$55.00 if you played CYO Volleyball or Soccer
Additional Requirements:
· Knee pads
· Appropriate Size Basketball
Requirements to register:
· Original Birth Certificate and a copy of Birth Certificate
· Proof of residence
· Proof of Religious Education
Coaches and Volunteers are needed to help

Due to the deadlines that are set by the zone and archdiocese, we are unable to accept any late registrations with the exception of registering players to fill teams that are short. Please make sure you can attend one of the published registration dates listed on this flyer, otherwise your registration WILL NOT be accepted.
