To view and print the St. Francis de Sales Certificate Order Form, click on this: Certificate Program Order Form. To print your order form, choose file and print. Complete the order form. Send the order form and check, made payable to St. Francis de Sales, in an envelope to school. On the envelope, write "Certificate Program," and your child's name and room number. Please note: Students may bring home cards and certificates. Please designate the student name and room number on the form and sign the release. Since gift cards and certificates are used as cash, St. Francis de Sales School will not be liable for lost or stolen cards and certificates delivered in this manner.
