If you are new to the parish, please fill out the following census form. You may put it in the collection basket at Mass, bring it to the church office, e-mail it to espe@stjudeboca.org, or mail it to our office at 21689 Toledo Rd, Boca Raton, FL, 33433. The Parish Office is open from 8:30am until 5:00pm Monday through Friday.
Your registration verifies that you are a member of our Parish. This becomes important when family members are receiving the Sacraments of Baptism, Eucharist and Matrimony. Being a sponsor for Baptism here or in another parish requires registration in a parish.
