St. Matthew Catholic Church at 8015 Ballantyne Commons PKWY, Charlotte, NC 28277 US - Diocesan Policies for All Parish Volunteers

Diocesan Policies for All Parish Volunteers

 June 2009

Thank you for your interest in St. Matthew’s ministries.  We are happy to have you become an active volunteer of our faith community!  St. Matthew is committed to providing a safe environment for all members of our faith community, especially the young and vulnerable adults.  In accordance with the Diocese of Charlotte requirements, all parish volunteers must complete the following four (4) forms before they can participate in parish ministry.

  • Notification and Release Form authorizes the parish to request a background check.  The form is submitted to the Diocese of Charlotte by the parish for processing.
  • Volunteer Profile asks for standard personal information.
  • Acknowledgement of Receipt Form which is the last page of the Sexual Misconduct Policy handbook; this should be signed and dated.
  • Certification of Receipt Form from the Code of Ethics handbook; this should be signed and dated.  (Click here for Code of Ethics Receipt which may not be available on the Diocesan website)

Forms can be obtained at the receptionist desk during normal office hours.  You can also access the forms online at www.charlottediocese.org; the forms and handbooks are available from their home page under “How We Serve”, then select the “Human Resources” link.  Completed and signed forms need to be returned to the parish office in a sealed envelope to the attention of Lisa Hornung.  These can be dropped off at the receptionist desk or mailed to: St. Matthew Catholic Church, PO Box 49349, Charlotte, NC 28277.

Additionally, volunteers must attend a Protecting God’s Children workshop.  This program is intended to educate volunteers to recognize and prevent sexual abuse.  Pre-registration is required for the workshop.  You may do this online at www.virtus.org.  After the workshop, you will be asked to keep your training updated through monthly bulletins online at www.virtus.org.  The monthly training requires approximately fifteen (15) minutes.  There is an additional recertification yearly that is also online.

  • Please check the parish bulletin and website for upcoming dates of “Protecting God’s Children” at St. Matthew.  (Next PGC workshops at St. Matthew are Sunday, Oct. 4, 9:00am and Thursday, November 5th, 10:00am.  Register at www.virtus.org)
  • If you do not have access to the internet, or are unable to work online, please contact Lisa Hornung, Business Office Assistant, at (704)543-7677 ext. 1016.
  • Please visit the diocesan website for additional information about the “Protecting God’s Children” program.

For further information or questions, please contact Deacon Jim Hamrlik at (704)543-7677 ext. 1040.  If you believe you have previously completed the forms, or have attended a workshop, please contact Lisa Hornung at (704)543-7677 ext. 1016.
